新冠肺炎防疫期間請假說明

• 若曾出入CDC公告確診個案活動史地點，或被通知須實施居家隔離/居家檢疫/加強自主健康管理/自主健康管理時，請主動填報「具感染風險對象回報系統」，填完後衛生保健組會主動與同學聯絡，如經衛生保健組評估同學不宜到校，將主動協同生輔組協助處理（防疫）公假事宜。
• 如非上述狀況，而是同學對自身健康風險評估有個別需求而自行請假者，則請申請病假（請以細胞簡訊、快篩或就醫相關證明等為佐證）。
• 新冠肺炎疫苗接種請假：疫苗接種日當天起於3天內(含接種日)，請同學自行至網路請假作業申請公假，可逐日申請。事由輸入「疫苗接種」，日間部同學指導單位請選擇「生活輔導組郭瑄尹」，進修部同學請選擇「學生事務組鍾佩涵」佐證請上傳疫苗接種證明(小黃卡) ，超過3天者，除接種證明外，請同時上傳醫師診斷證明。如有疑問，請與生活輔導組／進修部學生事務組聯繫。
• 如在期中、期末考期間，申請 (防疫)公假者，務必事先(預約後)告知任課教師；專科部高年級及大學部同學，請與任課教師協商補考方式；四技共同英文修課同學，以及專一至專三年級同學須先至教務處課務組申請補考，核准後之公假補考成績依學則規定按實際成績計。

Dear students:

Leave of Absence during Coronavirus Pneumonia epidemic prevention illustrate:

1. If you have been to places where CDC announced confirmed cases or have been notified of the need for self-isolate /self-health monitoring /strengthened self-health management /self-health management, please take initiative to fill in the “reporting system for persons at risk of infection”(web:). After filling in the form, the Health Section will contact the students. If the Health Section assesses that the students are not suitable to come to school, they will assist the Student Assistance Section to deal with (epidemic prevention) official leave matters.
2. If the student does not have the above conditions, but has individual needs for own health risk assessment, please apply for sick leave (please use Cell broadcast, rapid test or medical-related certificates as supporting evidence).
3. Leave of Absence for Coronavirus Pneumonia Vaccination: Within 3 days from the day of vaccination (including the day of vaccination), students should apply for official leave by themselves at the Online Leave of Absence Operation, and can apply in daily basis. For the reason of "疫苗接種", please select "生活輔導組郭瑄尹" for day division students and "學生事務組鍾佩涵" for division of continuing education students. Make sure to upload the vaccination certificate (small yellow card) for supporting evidence. In case you got vaccinated for more than three days and still experience adverse symptoms, please upload the doctor's diagnosis certificate in addition to the vaccination certificate. If you have any questions, please contact the Student Assistance Section/ the Student Affairs Section of the Division of Continuing Education.
4. Please inform the teacher in advance (after the appointment) if you’re apply for (epidemic prevention) official leave during mid-term and final exam weeks. Senior students of five-year junior college program and college students, please consult with the professors for make-up exam arrangement; students from four-year college general English courses and first/second/third-year students of five-year junior college program should apply for a make-up exam at the Academic Affairs Section. Results of such make-up exams after approval will not suffer from a usual make-up penalty which will deduct your score.

6. The official Leave for Novel Coronavirus Pneumonia will not affect the qualification of the exam and the conduct of the grades.
